Access Family Navigator through Trillium Health Resources

ASNC is partnering with Trillium Health Resources for residents of Pitt, Beaufort, New Hanover, and Pender counties to provide support to individuals on Medicaid through Family Navigators.

What do Family Navigators do?

Family Navigators – who are all parents of loved ones with intellectual and developmental disabilities (I/DD) or traumatic brain injury (TBI) – teach families how to navigate systems and access resources. From the Medicaid system to the school system to the medical system, a Family Navigator walks beside a family, attending meetings, assisting with paperwork, and providing connections. They can assist individuals and families with life transitions and help with benefits applications. Learn more by watching Trillium’s video.

What is the process of working with a Family Navigator like?

After the Family Navigator verifies eligibility, they work with the individual and family to create a plan that supports their needs and goals. The Family Navigator can provide up to 10 hours of support each month to educate the family about systems, processes, and resources. The Family Navigator also facilitates meetings and connections to promote self-care, strengthen social connections, and decrease isolation. Through challenges and successes, the Family Navigator is a partner in helping loved ones achieve their full potential and live successfully in the community.

Who is eligible to be supported by a Family Navigator?

This service is for individuals who are 3-64 years old who reside in Pitt, Beaufort, New Hanover, or Pender County. Individuals must have Medicaid and any I/DD or TBI diagnosis. Individuals on a Medicaid Standard Plan or Innovations Waiver are not eligible for this service.
